If you're trying to backstab an enemy in a stun/stagger animation, for instance, after breaking its poise, no backstab. Everything else is positioning. If you obey these rules and don't get a backstab, you weren't in position. Honestly, backstabbing is the easiest and most broken aspect of Dark Souls' combat system.
